When you come to Dentistry of West Bend for your first appointment, you will be greeted by our friendly office staff who will request that you fill out some intake forms and provide us with your insurance information if applicable.

Your first visit with Dr. Harding will typically include a full digital X-Ray of your mouth, a comprehensive examination, including a periodontal examination, and a discussion of your family medical history, as well as any other information Dr. Harding may require. Once Dr. Harding has made his assessment, he will sit down with you to discuss his findings and prepare a customized treatment plan just for you.

What should I bring to my first appointment?

You can help our staff to better expedite your file by bringing the following documents and information to your first consultation:

  • Patient Registration Form / Medical History
  • Dental insurance ID and completed insurance forms if applicable
  • Prior X-rays taken by another dentist or physician, if applicable.
  • A list of all medications that you are currently taking

If you are unable to download our patient intake forms and fill them out in advance, we request that you arrive a few minutes early for your appointment to allow time to fill those forms in our office.

Please Note: We request that all patients under the age of 18 be accompanied by a parent or guardian.

Please inform our staff if you have any existing condition that may affect your diagnosis and treatment, such as:

  • Diabetes
  • High blood pressure
  • Lupus or other disorders of the immune system
  • A pacemaker
  • Artificial heart valves
  • Artificial joints
  • Rheumatic fever
  • Prior dental work
  • Other medical conditions

Also inform us of any medications you are taking currently, including prescription medications as well as over-the-counter remedies.

What if I don’t have my prior X-Rays?

If your dentist or physician has recently taken X-rays of your mouth, you can request that they forward these to our office or provide us with the doctor’s contact information and we will contact them to make the request. If Dr. Harding requires additional X-rays, we can take them in-house.
